How to Prepare for Your Appointment

To ensure you make the most of your visit with Dr. Chopra, please follow these suggestions.
An initial evaluation process is usually 7 hours long. Dr. Chopra spends 1 hour the day before the appointment reviewing all the paperwork. On the day of the first evalaution it can take between 4 hours to 5 hours depending on the complexity. He then writes a very detailed plan of management which takes about 2 hours. Dr. Chopra will have already reviewed your medical timeline and forms on the day prior to your appointment to prepare for a thorough evaluation.

Managing complex medical conditions is like assembling a jigsaw puzzle—it requires gathering all the pieces to create a complete and accurate picture of your health. Every piece of information is vital for connecting the dots. - Create a Medical Timeline
- Why It’s Important: A medical timeline is the most helpful tool you can provide.
- How to Prepare: Include as much detail as possible about treatments, symptoms, and doctor visits. If you can’t recall exact age, approximate details are fine. For each treatment, describe the outcome. For example: “I tried medication X, and it helped reduce pain,” or “I had surgery Y, but it did not resolve the issue.” We do not need names of doctors. The timeline is not just a list of medical conditions, its a about your life by age.
- Tip: The more detailed your timeline, the better Dr. Chopra will understand your condition.
- Please do not use unknown abbreviations, do not use hyperlinks
- Please send the timeline in MS Word or Pages. Do not send it as a pdf, excel or any other format
- Include the legal name of the patient in the subject when emailing us.
- We need the medical timeline and the office forms at least 4 weeks before your appointment.
- Complete Pre-Appointment Forms
- These forms are basic and should not take much time to fill out. Please complete and send it to us.
- Please use the patient's full name whenever you communicate with us. We have no idea who "munchkin" or "lizzy" is.

- Bring a Support Person
- Having someone with you can be invaluable. They can help take notes, ask questions, and ensure all important details are captured.
- If you come alone, use a notepad or laptop (instead of a smartphone) to jot down important points.
- If your support person is going to type notes (we do not recommend that) then please type silently. This is a time when Dr. Chopra is focussed on the patient and its best to avoid any distractions
- Wear Comfortable Clothing
- Loose-fitting clothes make it easier for Dr. Chopra to perform the physical examination.
- Bring Comfort Items
- If you find comfort in a favorite pillow, blanket, or stuffed toy, feel free to bring it along.
- Manage Fatigue
- If you are prone to fatigue, let Dr. Chopra know. You are welcome to lie down during your consultation if needed.
- Communicate Symptoms Clearly
- Share all symptoms, no matter how minor they seem. Be open about anything causing you discomfort or concern.
- Highlight major concerns so Dr. Chopra can prioritize them.
- Prepare Questions
- Write down any questions you have and bring them with you. There will be ample time to discuss these during your visit.
- Be Assured of a Safe Space
- Dr. Chopra understands that patients with complex conditions often face challenges like being dismissed or misunderstood. He will listen to your concerns carefully and incorporate them into your treatment plan.
