1. Duration of the Evaluation
The initial evaluation for complex medical conditions may last between 4 to 5 hours, depending on the complexity of your medical situation. Dr. Chopra will have already reviewed your medical timeline and forms on the day prior to your appointment to prepare for a thorough evaluation.
  2. Required Documents
To ensure a smooth and comprehensive evaluation, please send the following to the Center for Complex Conditions at least 15 days before your appointment:

    • Your completed medical timeline
    • Any required forms
Submitting these documents on time allows Dr. Chopra to prepare adequately for your visit.
  3. Note-Taking
It is highly recommended to bring someone with you who can take notes during the appointment. If this is not possible, please come prepared to take notes yourself using either paper or a laptop (not a phone, as it may be cumbersome).
  4. Comfort During the Appointment
Since the evaluation can be physically and mentally demanding, you are encouraged to bring snacks or fluids to help you stay comfortable and maintain energy. Coffee and tea will also be available upon request.
  5. Prioritizing Your Concerns
At the start of the evaluation, Dr. Chopra will review your primary medical concerns in a list format. This step helps prioritize what issues need immediate attention.
  6. Comprehensive Examination
Dr. Chopra will conduct a detailed, head-to-toe evaluation, including:

    • Asking questions to gather information about your symptoms and medical history.
    • Performing a thorough physical examination of each part of your body.
  7. Medication Review
Your current medications will be reviewed, including their dosages and effectiveness. Dr. Chopra will assess whether they are helpful or not.
  8. Break Period
After the examination, there will be a short break lasting 15 to 20 minutes. This allows you to rest and prepares you for a check of your blood pressure and pulse rate.
  9. Detailed Discussion of Your Condition
Dr. Chopra will provide an in-depth explanation of your medical condition(s), including the reasoning behind his diagnosis. This part of the evaluation is collaborative, with you being the most important member of the care team. Your input, questions, and concerns will be thoroughly considered and addressed.
  10. Management Plan
A detailed management plan will be discussed, outlining the next steps for your care.
  11. Questions and Clarifications
At the end of the evaluation, Dr. Chopra will address any remaining questions, concerns, or clarifications you may have to ensure you leave the appointment feeling informed and supported.
